IT System Upgrade: Please Help Us by Updating Your Information

We have now upgraded to a new IT system and medical software, which will help us communicate more effectively with you - especially via SMS and email. Thank you for your understanding and patience during this transition while we are still adjusting to the new system.


Complete Your Patient Details Update Form

To ensure we have the most up-to-date information for your care, we will be asking all of our patients to complete our Patient Details Update Form. We understand filling out forms can feel like a hassle, but having the right information on file helps us to provide a more accurate and efficient service. By completing this form, you’ll help us:

  • Verify Personal Details - To make sure your contact details, any concession cards, and emergency contacts are correct.

  • Identify Authorised Contacts - To let us know who is allowed to discuss your medical information on your behalf.

  • Record your SMS Consent - To send important reminders and clinical information via SMS.

  • Facilitate MyMedicare Registration - To make it easier for you to register with us as a MyMedicare patient if you haven’t already.


How to Fill Out the Form

We have made this process simple by digitising this form through HotDoc. If you use HotDoc to book appointments, you will be prompted to complete the form online when you book your next appointment. You will only need to complete this once. If you prefer a paper version, we will have one available at our practice for you to fill out in person.
